Comments:

Sleeve: Probably a plain sleeve of some kind.

General: From Greg Russo:

"I have the master tape of the second Johnny Fortune single, and "Gee, But I Miss You" should not have been the A-side as shown on the record on your site. The A-side should have been "I'm In Heaven (When I Kiss You)." I have listed the record in the box set as the way it should have been released. In addition, the record was reviewed in Billboard magazine with "I'm In Heaven" as the A-side.

The "-1" side of an Emmy record number is the A-side, while the "-2" side is the B-side. I don't know why that Johnny Fortune record was not pressed correctly. Someone made a mistake along the way."
